INTERNALMEDICINE
COMMONCOLD
Thecommoncoldisadisorderwithnasalobstructionanddischarge,
sneezing,headache,cough,fever,chillinessandmalaiseasitschief
manifestations.Itusuallyoccursinwinterandspring,butmayalsobe
seenthroughouttheyear.Influenzaisalsogroupedunderthiscategory
intraditionalChinesemedicineandtreatedaccordingtotheprinciples
mentionedbelow.
EtiologyandPathogenesis
Thisdiseaseismainlyduetotheinvasionofexogenouswindand
usuallyoccursduringasuddenchangeofweatherwithunexpected
coldorwarmth.Inappropriateclothing,beingdrenchedbyrainand
fatiguemaydecreaseone'sbodyresistanceandpredisposeonetocatch
acold.Thepathogenicfactorsusuallyaredifferentinthefourseasons.
Forexample,wind-coldisthefactorinwinter,wind-heatinspring,
associationwithsummer-heatanddampnessinsummer,withdryness
inautumnandwithdampnessintherainyseason.Hence,accordingto
thevariationsoftheclimate,thepathogenicfactorandthepatient's
constitution,thediseasemaybemanifestedasawind-heatsyndrome
oradisorderassociatedwithsummer-heatoradampnesssyndrome,
whichmaychangeorcomplicateeachotherduringthecourseofthis
disease.However,nomatterwhatkindsofpathogenicfactorsare
involved,theyimpairthedescendinganddispersingfunctionsofthe
lungs,leadingtotheblockageofthesweatpores.Inthecaseofa
maladycausedbyseasonalpestilence,suchasinfluenza,thegeneral
symptomsareremarkableandsevereandmaybecomplicatedbyother
diseases.
SyndromeDifferentiationandTherapeuticPrinciples
A.Syndromedifferentiation
(a)Whenthepathogenicfactorislocatedinthelungsandbody
superficies,thedisorderisusuallyattributedtosuperficies-stheniasyn-
drome.
(b)Toidentifythenatureofpathogenicfactors,soastodistinguish
thewind-coldsyndrome,wind-heatsyndrome,andcomplicationof
summer-heatsyndromeanddampnesssyndrome.
B.Therapeuticprinciples
Eliminatingwind,releasingstagnatedlung-qiandexpellingsuperfi-
cialevilsarethemaintherapeuticprinciples.
ClassificationandTreatment
A.Wind-coldsyndrome
Manifestations:Severechillinesswithmildfever,anhidrosis,
headache,sorenessandpainofthelimbs,nasalobstructionordis-
charge,itchingofthethroat,coughwiththinsputum,nothirstorthirst
withdesireforhotdrink,thin-whiteandmoisttonguecoatingand
superficialorsuperficial-tensepulse.
Therapeuticprinciples:Expelsuperficialevilwithdrugsofacridtaste
andwarmnature,releasethestagnatedlung-qi,anddispelcold.
